My friend and colleague had a heart attack Dec 26 2018 at 4:00 PM. He is scheduled for a triple by-pass surgery next week. Oh my, that is harsh. I'm very sorry for your friend, Steve. His SSR for Concord, NC also has natal Saturn partile square MC, although solar Venus rises partile, and s.Jupiter is 1º11' from EP. Hopefully that means a speedy recovery, along with receiving great medical treatment, and care and emotional support from loved ones.
His Sun is also background in his SSR. From Jim's interpretations:
Transiting Jupiter will semisquare his Neptune from January 2, and contact several other planets like his Moon and Uranus as the month passes. Hopefully this gives him relief, and improvement in health and spirits. There's also a Venus-Jupiter conjunction connecting to his Moon on Jan 22-23 or so.

Excellent points Scales, particularly about the background Sun in his SSR and his Natal Saturn partile 90 SSR MC. I think he will actually be better health wise after he recovers from the surgery. I had another friend who had the same surgery and he felt much better after he recovered from the by-pass operation. It seems as one's arteries slowly become blocked, one starts gradually feeling worse heath wise not realizing its the unknown slowing blocked arteries that is making them feel bad, not solely old age, but then after a by-pass their heart receives more oxygen and they feel much better. My friend who has already had by-pass said he felt like he had a new body after his recovery from by-pass. I think the same will apply in this case with another friend having by-pass, and I told him he will feel better health wise after he recovers from the operation.

Steve, this is a great example. Sympathy to your friend: Though I think you're right that he's going to be fine (or better!), I know this is no fun to go through at the time.
Two transits are obvious stand-outs, and they're classics! Saturn squares his Sun within half a degree, then transiting Mars crosses an angle for an instant crisis. Text-book. However, notice it's also a great example of the effect of precession. If these transits were taken tropically, it would be like subtracting almost a degree (about 53') from all his natal positions. Neither Saturn-Sun nor Mars-Asc would then be in partile orb. It's a great example of how transits must be taken Sidereally (or, if tropically, then one must intentionally adjust for precession).
Two other transits draw my attention. First, in the swift-transit category, Venus conjoined natal Saturn (within 0°03'!). I've seen a lot of Venus-Saturn natal aspects for congenital heart disease, but not for heart attacks or later heart disease. Nonetheless, the metaphorical phrase "heart-broken" often applies very literally by occurring with heart trouble. This makes me wonder if some sort of emotional blow or deep sadness triggered this. (Otherwise, it was probably an existential moment of "Hell no, God, not now, I've still got plans, Dude!" or some such thing.)
The other, larger-scope transit of interest is Uranus opposite his natal Mercury-Neptune. I imagine this refers to a larger range of shifts in his life but, in terms of this particular event, the words that come to mind are "wake-up call," a sudden surprise that forces one to face reality instead of comfortable illusion.

On his surgery Demi, I'm not worried about the Mars. After all, he's having surgery. It fits the known conditions.

On further digging, I see that, at the moment of the heart attack, his natal Pluto was crossing local IC within about a degree. That's the existential confrontation. (Similarly, progressed MC is about to move into partile conjunction with progressed Pluto and is currently 19' from conjunct natal Pluto.)
Transiting Mercury was exactly setting. Not sure what that means, and I don't want to slip into the "easy answer that doesn't mean anything" of saying this was all a message to him.
His SSR was unusually nice this year. Venus rising, Jupiter on EP. Uranus 25' from natal Moon is most interesting, though not strongly placed: Still, at that orb, it's going to be felt, and probably is what you referred to as "a whole new life" disclosed (though it's tempting to interpret it as stress, too).
Here's a really clean hit: SNQ MC was 1°16' Scorpio for the heart attack, with progressed Saturn 1°53' Scorpio. If the PSSR (mean calculation) was at all relevant it was for the relatively happy outcome, because its Asc hits both his natal and transiting Jupiters.

I've wondered for a long time whether there was an equivalent "Novienic Lunar Return" that parallels the Ennead but for Moon instead of Sun. If the one for Sun works so well, why not Moon? My friend’s by-pass surgery occurred between 7:20 AM-11:30 AM yesterday, everything is fine. Early yesterday evening I get a text from his wife and she said my friend said to tell me:

My friend was referring to the time when he and I worked for the same Theatre company as young adults and he fell out of a large loading truck while we were loading projection equipment. He fell flat on his back slamming his head to the payment and I thought he had killed himself.
