Crashed into the Atlantic south of Nantucket Island, MA after departing JFK. All 217 people aboard died. The proposed reasons for the crash are complicated and disputed. After going into a dive, the craft broke apart mid-air at about 10,000 feet.

The lunar ingresses make me think that the mystery of the crash is what we see best - with a possible Jupiter-Neptune theme of "misdiagnosis."
