Apr 27, 1976, about (just after) 10:00 AM, Willow Island, WV (39N22'02'', 81W17'29'')
A cooling tower, "under construction at the Pleasants Power Station at Willow Island, WV," collapsed killing 51 construction workers. "Thought to be the deadliest construction accident in U.S. history." Details of the "dominoes falling" collapse are given in the article.

This may not fall in the usual category of "structural collapses.": It occurred during construction and, at the very least, is quite different from any of the other "structural collapse" events in the collection. It's important anyway because it's the deadliest industrial accident in U.S. history, but I'm not clear how to categorize it. I'm going to keep it around for a while to dig more into it.

The collapse occurred as Neptune squared MC, with Moon near square to Mars but - closest of all - an exact Sun-Jupiter conjunction.
