Japan Airlines Flight 123

Deadliest single-aircraft accident in history, and the second deadliest aviation accident ever.
Explosive decompression from a burst bulkhead at 6:24, fell off radar at 6:56 and crashed moments later.

"In-flight structural failure due to maintenance errors in 1978, which caused loss of flight controls." 520 died, 4 survived (injured).

It "suffered an explosive decompression 12 minutes into the flight and, 32 minutes later, crashed into' a mountain 60 miles from Tokyo. "The death toll was exacerbated by delays in the rescue operation."

The charts aren't great, but they're adequate. They're fine for explosives, but atypical for air travel accidents.
